- কীভাবে ভাই?

- কার্বন ডাই অক্সাইড কমে গেলে ফ্রি বাইকার্বনেট বেড়ে যায়, যা ক্যালসিয়ামের সাথে বাইন্ড করে আয়নাইজড ক্যালসিয়াম কমিয়ে দিয়ে Tingling, numbness করতে পারে। উনাকে আপাতত রিব্রেদার মাস্কের মত পলিথিনে শ্বাস নিতে বলো, কার্বন ডাই অক্সাইড বেড়ে গেলেই ঠিক হয়ে যাবে।

- আপাতত কী ম্যানেজম্যান্ট দেওয়া যায়?

- ক্যালসিয়াম ট্যাবলেট আর গাবা পি খাইতে দাও।

ভাইয়া, চাচার দুই দিকে নয়, শরীরের কেবল এক দিকেই নাকি ঝি ঝি লাগছে।

- কী বললে, একদিকে? তাইলে এটা তো Transient Ischemic Attack আই মিন Pre-stroke condition, যেটা কনফার্ম করতে গেলে MRI DWI film লাগবে।
.
সেই রাতেই রোগী ভর্তি হল ল্যাব এইডে।

দেখা গেল ধীরে ধীরে রোগীর বিশাল স্ট্রোক ডেভেলপ করল (evolving stroke)

⬛ Analysis
.
Tingling numbness can occur due to -
.
➡ Parietal lobe cause - unilateral
.
➡ Thalamic cause - unilateral [as thalamus is the common sensory pathway]
.
➡ Spinal cord cause - Uni/Bi lateral [Due to involvement of anterior & lateral column: Small fiber neuropathy]
.
➡ Peripheral neuropathy - Asymmetrically bi lateral
.
➡ Hypocalcemia [As sodium permeability is inversely proportional to Ca conc.]

⬛ Two types of onset: Gradual & Sudden
.
✌ Sudden onset Tingling & Numbness are -
.
➡ Hypocalcemia (Bi lateral)
.
➡ Compression neuropathy e.g. carpal tunnel syndrome (focal tingling & numbness which can be uni/bilateral)
.
➡ Radiculopathy - focal tingling numbness
.
➡ Cord hemisection (Uni lateral) &
.
➡ Stroke - involvement of parietal lobe/thalamus (Unilateral)

⬛ সুতরাং #Sudden #global (not focal) #unilateral tingling numbness can be first feature of stroke, if spinal cord cause can successfully be excluded.
